The Internet Questionnaire Sampler section introduces different questionnaire designs to be used in the Netscape browser and the E-mail environment to solicit information from the Internet users. This section also includes a collection of Internet Questionnaires examples currently used by companies, universities, and other organisations.


Using HTML for Questionnaire Design

Here are some of the HTML codes one could use for questionnaire designs. A major advantage is that information can be collected from users through an ordinary Web browser, such as Netscape's Navigator. In addition, most users are already familiar with the browser interface, such as buttons, drop-down menus, and option lists, which makes them even more user-friendly.

Drop Down List This is what a drop-down list (or sometimes called pop-up selector) looks like. User could only select ONE option from the pre-defined list.

For example, the above list allows user to pick a favourite fruit (from the list of five), with Kiwifruit pre-selected.

Radio Button Like a drop-down list, users could only choose ONE of the options avaiable from the list. Again, using the fruit example with Kiwifruit pre-selected.

Single-Line Input A single-line input is usually for name inputs (SIZE=20 and MAXLENGTH=30) such as:

Text Area A text area on the other hand has multiple rows with horizontal and vertical scroll bars, usually for user comments.
The example below is a text-area with COLS=50 and ROWS=3.

Multiple Selections This time, users could choose one or more options from the available list, such as:

To select, click the appropriate choice(s) while holding down the Ctrl (Control) key.

Only four lines are visible at any one time because of the HTML code SIZE=4.

Check Box Check box is very similar to multiple selections, where users could check (tick) all the boxes applied.

Questionnaire Examples

MicroVision MicroVision's Lifestyle Game is an example of how to use geodemographics (e.g., combining postal codes with income levels) for consumer segmentations.
SRI's VALS Using two dimensions: self-orientation and resources, SRI's VALS-2 (Values and Lifestyles Program) defines 8 segments of adult consumers who have different attitudes and exhibit distinctive behavior and decision-making patterns.
This web site also includes VAL-2 cases, the actual VALS-2 Survey, and a section for non-US users.
GVU's WWW User Survey Since January 1994, the Graphics, Visualization, and Usability (GVU) Center at Georgia Tech has been conducting the WWW User Survey.

Survey Assistant WWW Survey Assistant is a program that has been designed to simplify the process of World-Wide Web survey publishing. Such as the ability to create a user designed project and automatically transformed it into HTML format and CGI program.
Internet Questionnaire System Concepts Ltd's Internet Questionnaire is a survey for the British Standards Institute to identify what users want from on-line information.
Giftworld: Questionnaire Giftworld Questionnaire is an online suggestion box by Giftworld, a Dutch mail order company offering handmade products.
LifeNet: Questionnaire LifeNet Questionnaire is an online survey with mostly financial-planning questions for its potential clients.
Technology Panel IntelliQuest's Technology Panel is a worldwide research panel of technology users. This panel is a collection of people who are using technology products at home or at work. You could also check out their Member Profile Survey.
Skin Care Survey PSS InterNet Skin Care Survey is another example of how to use the Web environment to collect information from potential customers.
